The spread is the point. A membership at the bottom of this table and one at the top differ by more than an order of magnitude, and neither figure tells you what the medicine costs.

What a published price usually excludes

The medicine
Almost always a separate line. Where a clinic dispenses in-house or through a partner pharmacy, ask for the medicine cost in writing, separately from the programme fee.
Laboratory work
A baseline panel before initiation, and repeats during treatment. Not one clinic in this survey published a laboratory price, so we report insufficiency rather than an estimate.
Follow-up
Some memberships include review visits; others bill them. This is the most common source of a bill that exceeds the advertised figure.

GLP-1 medicines are priced separately from clinic fees

A price difference that is really a regulatory difference

A markedly cheaper GLP-1 offer usually means a compounded product rather than the approved one. Compounded drugs are not FDA-approved and are not reviewed by FDA for safety, effectiveness or manufacturing quality — and FDA has stated that semaglutide and tirzepatide no longer appear on its shortage list.   • What does the advertised figure include, and what is billed separately?
  • What does the medicine itself cost, per month, at the dose you would start on?
  • What laboratory work is required before starting, and what does it cost?
  • Is the medicine FDA-approved for this use, off-label, compounded or investigational?
  • Are follow-up visits included in the membership or billed per visit?
  • What happens to the fee if I stop treatment mid-term?

Common questions

How much does peptide therapy cost?

There is no national figure worth quoting, because most clinics do not publish one. Across 190 clinics in 79 metros, 38 published any starting price at all. Where a price was published it described a membership or programme fee, and in every case medication, laboratory work or both were billed separately.

Is peptide therapy covered by insurance?

Generally not. Coverage depends on the specific medicine and the indication. An FDA-approved medicine prescribed for its approved use is sometimes covered; compounded and investigational products generally are not. Tesamorelin for HIV-associated lipodystrophy is the notable approved exception. Ask the clinic to confirm in writing before treatment.

Does this vary by city?

It does, and not in the direction you might expect. Why do published prices exclude so much?

A programme fee typically covers the consultation and the clinic’s oversight. The medicine itself, the laboratory panel before initiation, and follow-up visits are frequently separate line items. A "from $X per month" figure is therefore a floor, not a total.
